Immunizations and forms on the patient portal
/With summer approaching and summer camps gearing up, and summer jobs and internships starting, one common request we get is for copies of immunization records and school forms. These can be obtained right from the patient portal at any time! However, it’s not obvious at first glance how to get them, so we thought a primer would help!
For immunizations:
log into the patient portal
in the left side navigation, pick “Chart”
for the “include information from” drop down, pick “entire history”
scroll down and check the box for “immunizations”
scroll down further and you can choose to download the document or email it
School and daycare forms are generated at the time of a well child exam and sent directly to the portal, so you should get a message about it, which you can follow to retrieve it. However, you can also find it under that same “Chart” tab, under that pick “Clinical Notes” and you will find the most recent school forms (titled “NYS School Health Exam Form”) or daycare forms (titled… wait for it… “Day Care Form”)
